Although an uncommon finding, if there is also high signal on T2-weighted images, then you can make the diagnosis of a ayurvedic tear, as this is the result of synovial fluid leaking into a meniscal tear. If horizontal tears go all the way from the apex to the outer margin of the meniscus, they may result in the formation of a meniscal cyst. The synovial fluid runs peripherally through the horizontal tear and accumulates within the meniscus and finally result in a cyst. The lateral meniscus posteriorly comes up high over the tibial spine to insert near the posterior cruciate ligament. De rode pijl geeft een focale chondropathie graad iv aan op het tibiaplateau die na behandeling van de meniscus scheur nog klachten kan veroorzaken. In enkele gevallen duurt de zwelling na een operatie langer dan gemiddeld. Normaliter kan er tot 6 a 8 weken sprake zijn van een reactieve knie. Ook hiervoor is vaak de kraakbeenconditie verantwoordelijk.

Een degeneratieve meniscusscheur is vaak het eerste verschijnsel van een arthrose van de knie! De operateur kan voorafgaand aan een operatie niet altijd met zekerheid bepalen hoe groot het aandeel is in de klachten op basis van het meniscus letsel en dat van de eventueel aanwezige kraakbeenafwijkingen. Dit is dan ook de reden dat na een meniscectomie de mechanische klachten veroorzaakt door de meniscus meestal zijn verdwenen en soms duurinspanningsafhankelijke klachten voortduren, veroorzaakt door een verslechterde kraakbeen conditie. Op de foto's is goed te zien dat naast een degeneratieve meniscus scheur ook behoorlijke kraakbeen beschadigingen aanwezig zijn in het mediale compartiment van de knie. Deze patienten zullen hoogstwaarschijnlijk niet klachtenvrij zijn na de behandeling van het meniscusprobleem. .

Een gedeelte van de zo belangrijke schokdemper in de knie is namelijke (gedeeltelijk) verwijderd. Zelfs verwijdering van een klein segment van de meniscus kan de schokdempende werking van de meniscus in negatieve zin beinvloeden. De termijn waarop klachten ontstaan bedraagt ongeveer 15 tot 25 jaar. Het optreden van klachten hangt ook af van het activiteiten niveau (werk en sport de stabiliteit van de knie en de belastings-as (O of X-been). Bij de degeneratieve letsels van de meniscus is in het merendeel der gevallen ook sprake van in ernst wisselende kraakbeen beschadigingen, zeker bij de iets oudere patient ( 40 jaar).

Bij een traumatisch letsel van de meniscus bij een jonge patient is meestal geen sprake van een beschadiging van het kraakbeen. Het gescheurde meniscus gedeelte wordt verwijderd en dit leidt in het merendeel der gevallen tot een goed belastbare knie. . De grootte van het verwijderde gedeelte van de meniscus kan wel gevolgen hebben op de langere termijn (zie ook bij meniscus). . Immers met het verwijderen van een groot gedeelte van de meniscus wordt de druk op het kraakbeen vergroot.
